During the intake process of each new home health referral, at least one or more care type should be entered. Care types can be added in the Referral Source tab of the referral screen. Multiple care types can be added and can be added based on the reason for admission and primary diagnosis.
Care types are added by selecting 'Add Care Type' and selecting from the pre-populated list. Each care type must have an effective from date (today's date). The effective to date can be left blank unless other wise specified. Primary care types can be indicated by selecting 'Set to Primary' (shown below). The default care type will then be deleted by selecting 'Delete Care Type'.
Review Referral for Accuracy and Care Type Entry
This task, previously 'Review/ Approve Referral After Payor Verification', is routed to the Intake Supervisor and should prompt them to ensure a care type has been entered, along with their normal review.
Patient programs can be added as necessary based on a patient care type. Patient programs are located on the 'Clinical' tab of the referral and can be added by selecting 'Add Patient Program' and choosing from the pre-populated list. Multiple patient programs can be added. Each patient program must have an effective from date (today's date). The effective to date can be left blank unless other wise specified.
The workflow task 'Review Care Type and Assign Patient Program Accordingly' will generate when certain care types are assigned. This should prompt the Intake Supervisor to add the Patient Program as indicated by the care type.
Patient programs have a short description tied to them and are available for review in PointCare to indicate to the clinician that this patient is involved in a special program. Any patient with a patient program added will be indicated in PointCare by a blue "i".
